Before registering to the QReserve, Simon Fraser University requires your consent to store your personal information outside Canada. Please read the information below carefully before deciding whether to click the "I consent" or the "I do not consent" button at the bottom of the notice.

If you choose to click the "I consent" button, your consent is effective immediately and you will be linked to the QReserve where your personal information will be collected.

If you choose the "I do not consent" button, you are unable to use this service.

The Centre for High-Throughput Chemical Biology uses a service located outside Canada to track equipment and consumable usage within the facility. The personal information you will be asked to provide on QReserve includes:

- your name and email address

- your supervisor's name

- your lab location and phone number

If you consent, your personal information will be collected and stored on data servers located in the United States of America. Before giving consent, please review the QReserve terms of use / service and privacy policy.

If you consent, your personal information is collected under the authority of the University Act (R.S.B.C. 1996, c.468) and the Freedom of Information and Protection of Privacy Act (R.S.B.C. 1996, c.165). It is related directly to and needed by the University to track and manage equipment and consumable usage. The information will be used to invoice the appropriate grants for usages.
